Children In Entertainment Legislation - Required Information

In order that children can perform on stage within the legal framework, CAST. is required to seek approval from the Local Authority. This form seeks up-to-date information on your child(ren) and the data you provide will be used to complete our application to Northumberland County Council for the Five Year Anniversary Shows. Kindly provide the required information in line with the following requests.

If you have more than one child attending CAST., please submit a separate form for each child.
If the child IS PERFORMING in one or more of the shows, please provide their name and date of birth and complete all remaining fields.
If the child IS NOT PERFORMING in one or more of the shows, please ignore the reference to the school they attend, but complete all other fields.

Once you are satisfied that all sections are completed, please click on the SUBMIT button, which will email the details to CAST.
