CAST. STUDENT REGISTRATION FORM
Welcome to CAST. (the Cowell Academy of Stage & Television.) Please use this form to register a child as a CAST. student. The personal information you provide will be retained in secure format and will help us keep your child safe whilst they are in our care and will help us to provide the best tuition tailored to their age, experience and ability.
The very nature of performing arts means that at some times we will take audio and/or visual images of the students and some images will be published online for a variety of reasons.
We ask for details of medical conditions, allergies, dietary restrictions, disabilities, etc., so that we can deal with any issues that arise whilst your child is with us. If you consider any of the information we request is unwarranted or too sensitive to provide, then please speak with us before completing the form. All of our coaches and other adults working with us have got recent Data Barring Service (DBS) certificates that are free of any cause for concern.
